Edit: Not drug stylee Heroin;but there is a chemical in it that is a sub-division of morphine.

Just to clarify, "Heroin" was the brand name applied to diacetylmorphine by its creator, Bayer Pharmaceuticals (yep, the aspirin ppl invented heroin in the late 19th century and originally marketed it as a non-addictive alternative to morphine!). Bayer's marketing claims of course proved false, and while there was some abuse, it was outlawed in a few countries, largely because of an undeserved (and by that I mean inaccurate) rap; you see diacetylmorphine is just acetylated morphine, an opioid drug and a true narcotic. It is in the exact same class of drugs (opioids) as the Vicodin (hydrocodone HCL) that House is already addicted to. After Bayer stopped marketing the drug, it went underground, by and large because it is quite simple to synthesize from plain morphine which is found naturally occurring in the opium poppy. In past episodes we saw House using morphine which is very similar to heroin on the opioid scale of things, yet with some important differences. As with all other opioids (including House's drug of choice, Vicodin) heroin is converted to morphine in the central nervous system after it crosses the blood-brain barrier. The difference between heroin and plain morphine is that heroin crosses the barrier at twice the rate, resulting in a more powerful euphoric surge of well being after use.

So, House's use of heroin is really not any stretch from his use of Vicodin or morphine. Either will alleviate the withdrawal symptoms of the other, they produce very similar pharmacological effects, virtually identical in many respects. Vicodin (hydrocodone) is much "weaker" on a mg per mg basis when compared to heroin and it does not have the same spike of euphoria that heroin does, especially when the heroin is injected IV and thus hits the brain in seconds as opposed to 30 minutes via oral administration, so I suspect that with his growing pain, increased Vicodin intake (Vicodin is not pure hydrocodone, it is adulterated with acetominophen as well which is hepatoxic in high doses) and his colleagues unwillingness to prescribe to his needs, he makes the decision to switch to heroin. While it cannot be legally used, even in medicine in the United States (or here in Canada), it is used in many countries when legally prescribed by a physician (it is commonly used on terminal cancer patients in the United Kingdom under the brand name "Diamorphine" as it is found to be a superior analgesic (pain killer) than morphine, it has much fewer attenuate side effects and its associated euphoria spike is very helpful with elevating a terminal cancer patient's obvious inclination towards depression and despair - it is very calming).

Anyway, yeah, I just wanted to point this out, that the leap from hydrocodone and morphine use to diacetylmorphine is really nothing. Heroin has been ascribed this special "evil" status in North America because its only use here is illegitimate use by abusers only - it has no legitimate use in medicine here. In fact, as I illustrated, heroin is from a chemical and pharmacological point of view, no different from hydrocodone (Vicodin) and morphine (or other commonly prescribed opioids such as oxycodone [brand name OxyContin] or hydromorphone [brand name Dilaudid]). In fact, Dilaudid, a commonly prescribed opioid in the United States and Canada, is actually about 5 times stronger than heroin is on a mg per mg basis, and there are others stronger still (Fentanyl, about 80x stronger than heroin).

How do I know this? I was an addict, a functioning opioid addict for two years (by functioning I mean I still functioned within society, held a job, maintained relationships, paid my credit cards, all that stuff, all the while concealing my "secret life" as an addict - I am even educated and hold two bachelors degrees in Computer Science and Mathematics). I have taken a large variety of prescription opioids, such as OxyContin, Dilaudid and morphine and yes, the non-prescription opioid heroin as well. I will have been clean a year in 3 weeks time!!! :D

Anyway, I hope this clears all of that up.
