That looks like the vintage format. Did आप accidentally click on the link to use that uploading method? या was it a temporary glitch? I've just added an image and the normal method worked fine for me.
As Flicker mentioned, that's definitely the old format. आप probably accidentally toggled it somehow. It should go back to the newest one if आप leave the image uploading tab and go back to it.
